Long-Term Side Effects of Kratom You Might Overlook

Woman looking at her reflection in a mirror

Quick Answer: The most common long-term side effects of kratom are physical dependence, ongoing constipation, and changes in appetite or weight. They build so slowly that they’re easy to miss. Liver injury is rare, and when it happens, it usually appears within the first few weeks of regular use, not after years.

Not every change happens suddenly enough to get your attention. Constipation can start to feel normal, your appetite may slowly shrink, or you may sense that something about your health has shifted without knowing why. When kratom has been part of your routine for months or years, it can be difficult to recognize which changes may be connected to it.

If you’ve been wondering whether kratom is bad for you long term, this guide walks through the long-term side effects of kratom that are well documented, the ones that are rarer, and the ones that are still uncertain.

And if you’re curious about treatment, reach out to us at Casa Capri Recovery for a confidential conversation about your options.

What Are the Side Effects of Daily Kratom Use?

The side effects of daily kratom use can include recurring digestive problems, nausea, changes in appetite, and fatigue. Some people experience these symptoms regularly, while others report few noticeable problems or feel that kratom helps them manage pain, energy, or daily responsibilities.

The difficulty is knowing when a recurring side effect deserves more attention. A symptom that shows up most days can slowly start to feel like part of normal life, even when it’s a sign that something in your body has changed.

What Are the Long-Term Effects of Kratom?

The long-term effects of kratom are still being studied, and the available evidence is stronger for some concerns than others. Much of what we know comes from surveys, observational studies, and reports involving individual patients, which makes it important to distinguish recurring symptoms from complications that haven’t been firmly established.

Commonly Reported Effects

Constipation, nausea, and reduced appetite are among the more frequently reported physical effects. Kratom’s effect on the body’s opioid receptors can slow digestion, which helps explain why kratom constipation and other digestive problems are so common. With continued use, these symptoms may recur often enough to affect eating habits, comfort, and daily routines.

Reported With Prolonged or Heavy Use

Weight loss, tremors, and unusual skin darkening have been described in people using kratom heavily or over extended periods. Some medical case reports describe this kratom skin discoloration as blue-gray or brown patches on the face, neck, arms, or other areas, often on skin exposed to the sun, after years of use. These findings deserve attention, but they don’t establish that such changes are common among kratom users.

Effects That Remain Uncertain

Claims about permanent kidney damage, widespread cognitive decline, or lasting hormonal disruption go beyond what current evidence in humans can reliably establish. Individual medical reports have raised concerns about some of these complications, but we still don’t know how frequently they occur or whether kratom directly caused them.

When considering the long-term side effects of kratom, it’s worth remembering that a reported symptom doesn’t necessarily mean kratom caused it. Still, the lack of conclusive findings doesn’t mean years of regular use are harmless. The same is true for concentrated extracts and commercial 7-OH products, which have strong opioid-like effects but haven’t been studied over years of use.

Does Kratom Damage Your Liver Over Time?

Kratom liver damage is rare, but the timing may be surprising. According to NIH LiverTox, symptoms have typically appeared within one to eight weeks after people began using kratom regularly. Liver injury is therefore a concern even for someone who hasn’t been taking kratom for years.

Many documented patients experienced improvement after stopping kratom, although recovery sometimes took weeks or months, and some cases were severe. Researchers still don’t know why certain people develop liver problems while others don’t, or how frequently these complications occur among all kratom users.

Yellowing of the skin or eyes, dark urine, or significant itching may indicate liver problems and should be evaluated promptly by a healthcare professional.

Can You Have Kratom Dependence and Still Feel Fine?

Yes. Kratom dependence can develop while someone continues working, maintaining relationships, and handling everyday responsibilities. Feeling productive or generally healthy doesn’t necessarily mean your body hasn’t become accustomed to regular kratom use.

Findings from a 2024 study of 357 kratom users suggest that dependence can develop without obvious disruptions to daily life. More frequent use was associated with tolerance, withdrawal, and cravings, but not with greater difficulties at work, at home, or in relationships.

Someone might still feel that kratom is helping them get through the day, even as they become increasingly reliant on it to feel comfortable or function as usual.

Can Kratom Affect Your Memory or Concentration?

Heavier kratom use may affect certain aspects of memory, but there’s no conclusive evidence that regular use causes widespread cognitive decline. In one study, people who drank more than three glasses of kratom tea a day performed worse on a specific visual learning and memory task, while their results on other cognitive tests were similar to those of nonusers.

The study involved traditional kratom tea drinkers in Malaysia, so its findings can’t automatically be applied to concentrated extracts or other commercial products.

If you’ve noticed difficulty concentrating, forgetting things more often, or feeling mentally foggy, kratom may be one factor worth considering. Sleep problems, stress, medications, and other health conditions can also affect cognitive functioning, making an individual medical assessment more useful than assuming a single cause.

What Should Women Know About Long-Term Kratom Use?

Research on kratom side effects in women is still limited, because many kratom studies have included mostly men. That means some changes women notice, from shifts in sleep and mood to skin discoloration, haven’t been studied closely in women specifically.

Many women first turn to kratom to manage pain, anxiety, low mood, or exhaustion, often while caring for everyone else. When it works at first, it can be hard to notice that it has slowly become something you need.

Kratom use during pregnancy also carries its own risks, since newborns exposed to kratom have shown withdrawal symptoms after birth.

If you’re pregnant, planning a pregnancy, or breastfeeding, talk with your healthcare provider before stopping or changing your use.

How Do You Know When to Ask for Help With Kratom?

You don’t need to be certain that kratom is responsible for a symptom before bringing it up with a healthcare professional. Sometimes the clearest reason to ask questions is that something about your health or your relationship with kratom has gradually changed.

Consider seeking guidance if you’re experiencing:

  • Persistent physical changes: Constipation, appetite or weight changes, or other symptoms that continue affecting your health.
  • Unexplained health concerns: Unusual skin discoloration, signs of possible liver problems, or symptoms that don’t have an obvious explanation.
  • Growing reliance on kratom: Feeling increasingly uncomfortable without regular doses or finding it difficult to reduce your use despite concerns.

How Casa Capri Helps Women Stop Kratom or 7-OH Use

Many of the women who contact Casa Capri Recovery are still working, parenting, and keeping up with their lives when they first reach out.

A conversation with our team is a chance to talk through what you’ve noticed and find out whether treatment for kratom dependence could help, without any pressure to decide right away.

Casa Capri treats only women, in a home-like setting in Southern California. Care can include:

  • Medical detox to ease withdrawal safely after long-term kratom or 7-OH use
  • Residential care with individual therapy, group support, and trauma-informed care alongside other women
  • Dual diagnosis care for the anxiety, depression, pain, or trauma that often sits underneath long-term kratom use
  • Virtual IOP to keep building your recovery from home after residential care

The bottom line: Many long-term side effects of kratom are gradual and easy to dismiss, but they’re worth paying attention to. Some, like dependence and digestive problems, often improve once you stop, and you don’t have to figure out how to stop on your own.

FAQs About Long-Term Kratom Use

Is kratom safe to use for years if you haven't had side effects?

Not having side effects is reassuring, but it doesn’t rule out dependence or problems that develop without early warning signs. If you plan to keep using kratom, letting your doctor know allows them to watch for changes, such as in your liver tests, and to check for interactions with any new medication.

Kratom has been linked to a faster heart rate and higher blood pressure, and some case reports describe heart rhythm problems, but research hasn’t shown what years of use mean for heart health. If you already have a heart condition or take blood pressure medication, regular check-ups become especially important while you use kratom.

Long-term users have reported dry mouth and more frequent urination, along with constipation. These effects are usually more of a nuisance than a danger, but because they’re easy to blame on something else, they can go unnoticed as signs of regular kratom use.

Many effects, such as constipation and reduced appetite, often ease after kratom use stops, and physical dependence fades as the body readjusts. Some changes, including certain skin discoloration, may fade slowly or only partly, which is why it helps to have lasting symptoms checked rather than waiting for them to disappear.

A healthcare professional can investigate whether kratom may be contributing to your symptoms, but there isn’t one routine test that establishes it as the cause. Depending on your concerns, the evaluation may involve reviewing your medical history, examining your symptoms, ordering appropriate tests, and considering other possible explanations.
